The history of Czech Extraliga has its roots in the Czechoslovak First Ice Hockey League. The Czech Extraliga was created in 1993 following the administrative break-up of Czechoslovakia. Czech Extraliga is the highest level of hockey in the Czech Republic, and it is ranked as the third strongest league in Europe by the IIHF.

Cxech Extraliga is known for producing a lot of skilled and talented players that eventually play in the NHL. There are more then fifty players in the NHL from the Czech Republic.

Interesting aspect to the league is that the name of the league is leased to a general sponsor and changes frequently. In 1999-00, it was known as the Staropramen Extraliga. In 2001-01 and 2001-02 seasons, it was called the Český Telecom Extraliga. Rrom 2003-04 to 2005-06, it was known as the Tipsport Extraliga. And since the 2006-07 to the present, it has been known as the O2 Extraliga.

There are 14 teams that compete in Czech Extraliga. The top 10 teams at the end of the regular season advance to the final playoffs. After the regular season, the teams that finished 7th to 10th will play a pre-quarter final playoff series (best-of-five) to determine who will join the top six into the quarter finals (best-of-seven).

Since the 2006-07 season, three points has been be awarded for a win in regular time, and two points for an overtime victory, while the defeated team in overtime gets one point. If necessary, penalty shots are used to decide games after overtime.

Extraliga has a similar structure as other European leagues. The four lowest ranked teams, teams that are ranked 11 through 14 after the regular season play in a play out series. The last placed team after the play out series plays with a winner of 1. národní hokejová liga playoff series (best-of-seven).

The winner of this series will play in the Czech Extraliga next season. The loser will play 1. národní hokejová liga, which is the second level ice hockey league in Czech Republic behind Czech Extraliga.
